Twin Rivers Nursing & Rehabilitation has transformed its operations over the past three years, earning top federal quality ratings while expanding services for residents with complex medical needs, according to CEO and Administrator Robert Epley.
Located at 2420 W. Third St., the Owensboro facility has served the community since 1969. It operated under several names before becoming Twin Rivers Nursing & Rehabilitation in the early 2000s.
The facility provides long-term care, short-term rehabilitation, respite care, respiratory therapy, wound care, and specialized skilled nursing services. It also features one of the largest therapy gyms in the Owensboro area, offering physical, occupational, and speech therapy designed to help residents return home whenever possible.
Epley said Twin Rivers faced challenges related to quality measures and overall performance when he became CEO and administrator three years ago.
“We focused on placing the right people in the right positions and creating a culture centered on excellence,” Epley said.
Those efforts have resulted in a five-star Centers for Medicare & Medicaid Services quality measures rating and a deficiency-free Health and Life Safety Survey, he said.
Twin Rivers also has respiratory therapists on staff, an on-site wound nurse available daily, a wound care physician who visits weekly, and medical oversight from Medical Director Dr. Tamberly McCoy and Nurse Practitioner Amy Coy.
Epley said the facility has intentionally shifted its approach to resident care.
“We understand that while this is our place of employment, it is first and foremost our residents’ home,” Epley said. “That perspective guides every decision we make.”
He said the facility regularly cares for residents with complex medical needs that require a higher level of clinical support than many nursing facilities provide.
Epley credited frontline employees with driving the facility’s improvements.

He said the facility’s resident population continues to grow as more families seek its services.
