An Owensboro man died Tuesday morning in a crane accident at the Phillips 66 Refinery in Roxana, Illinois.
Madison County (Illinois) Coroner Steve Nonn identified the man as 47-year-old Chad L. Crabtree. Nonn said Crabtree was the project manager for Sterett Crane & Rigging, a subcontractor at the refinery.
“During the operation of the crane, a mechanical malfunction occurred and caused the crane to overturn,” Nonn said in a release.
The coroner said Crabtree sustained head trauma and was pronounced dead at the scene by Coroner’s Investigator Shelbi Frakes at 12:13 p.m. The coroner said Crabtree’s wife had been notified of the accident.
A second male worker, who was the crane operator, was also injured and transported to a local hospital.
The accident occurred at approximately 10:45 a.m. Tuesday, according to a statement from refinery officials.
The incident remains under investigation by the Roxana Police Dept and OSHA investigators
The emergency response team from the refinery also assisted at the scene.
Funeral arrangements remain pending at this time. An autopsy has been scheduled for Wednesday.
