Community Wellness offers remote patient monitoring telehealth program to Medicaid and Medicare participants

Remote Patient Monitoring allows Community Wellness to help clients proactively monitor their vitals and chronic conditions right from home. Through the program, eligible clients are provided with simple-to-use, FDA-approved, Bluetooth-enabled health devices at no cost.

RPM+ Program participants can track their vital signs and set up medication reminders in a secured application on their phone or tablet.
