Community Wellness, a leading company in health and wellness, is increasing access to care for Kentucky Medicare and Medicaid consumers through their Remote Patient Monitoring (RPM+) Program.

Remote Patient Monitoring allows Community Wellness to help clients proactively monitor their vitals and chronic conditions right from home. Through the program, eligible clients are provided with simple-to-use, FDA-approved, Bluetooth-enabled health devices at no cost.

RPM+ Program participants can track their vital signs and set up medication reminders in a secured application on their phone or tablet. Community Wellness’ proprietary software seamlessly integrates with all major Electronic Medical Record (EMR) systems to improve care coordination among all authorized providers involved in the patient’s care.

“What makes Community Wellness unique is our individualized coaching. In addition to tracking vitals, clients are also assigned a dedicated, highly-educated Health & Wellness Coach to work with them to meet their health and wellness goals,” said Jessica Pierce, Brand Ambassador.

Last week, Community Wellness opened a 24/7 Patient Monitoring and Training Facility in Danville, KY. The facility will serve patients in Kentucky and throughout the country by providing fully integrated remote patient monitoring for patients with chronic conditions, high-risk pregnancies, and opioid use disorder.

Community Wellness is the nation’s foremost innovator of evidence-based, high-touch, technology-enabled remote patient monitoring solutions. Community Wellness uses FDA-approved devices, certified health coaches, medical science, and best-in-class informatics to deliver improved outcomes, access, patient satisfaction, and lower costs to patients, physicians, hospitals, and health plans.
